Critical Provider is a term life insurance policy with an accelerated benefit rider that pays a lump sum benefit directly to you upon diagnosis of a critical illness or condition.

You can choose a full or limited benefit option and use that cash benefit to

  • cover health insurance deductibles or co-payments,
  • replace lost income,
  • take a recuperative vacation, or
  • cover any other needs to help you maintain your quality of life.

Plan Details

Critical Provider will accelerate a direct percentage of your term life insurance policy's death benefit for the following 18 diagnosed illnesses:

  • 100% Cancer
  • 100% Heart Attack
  • 100% Stroke
  • 100% Coronary Bypass Surgery
  • 100% Kidney Failure
  • 100% Major Organ Transplant
  • 100% Coma
  • 100% Paralysis
  • 100% Blindness
  • 75% Terminal Illness
  • 75% Nursing Home Confinement
  • 25% Benign Brain Tumor
  • 25% Alzheimer's Disease
  • 25% HIV Infection from Blood Transfusion
  • 25% Parkinson's Disease
  • 25% Aorta Graft Surgery
  • 25% Heart Valve Replacement or Repair
  • 10% Coronary Angioplasty

All benefits from the Critical Provider plan are paid to you regardless of any other insurance you may have. A 100% death benefit is payable if no other benefit listed above has been paid. All above conditions are payable one time only.

The Facts
